Home Health Care

There are two major types of home health care agencies. Some agencies provide non-medical help, including making meals, bathing, dressing, grocery shopping, transportation, dog walking, gardening, pet care, companionship, and the like.

Other agencies provide skilled nursing care, including hospice care to help people and their families cope with chronic terminal illnesses. Medicare will sometimes pay for short-term skilled nursing care. The net effect of home health care is keeping people thriving in the community in their homes, making hospitalization less likely. Services that provide Hospice information or care are asterisked (*). Many home health care agencies offer mental health services and services for dementia, as well.

Services: Aging Excellence’s quality in-home care is committed to the delivery of services and products that provide physical, social, and personal value to maximize
functional independence and increase socialization. Specifically, the care managers do the following:

Aging Life Care Management. The experienced, nationally certified professional Aging Life Care Managers specialize in assisting mature adults and their families with long-term care arrangements. The Aging Life Care Managers have experience in Gerontology, Social Work, Nursing or Counseling.

Home Services. Every person, Senior or Elder in the care of Aging Excellence goes through a detailed intake process ensuring that Aging Excellence is able to meet the person’s needs. The certified staff offers 24/7 on call services.

Handyman/Chore Services. The Handyman Services provide cleaning, packing, organizing, minor home repair, and yard services for those individuals who would like to spend their time and energy elsewhere. General cleaning tasks include vacuuming, dusting, mopping, laundry and linen change, bathroom and kitchen cleaning. Spring Cleaning includes heavy duty cleaning.
Social Companion/Homemaking. The Social Companion program provides caring companionship at a person’s home or during personal outings or errands.

Homemaking services provide help for individuals who desire assistance with home management tasks such as meal preparation, shopping, and errands.

Personal Care Services. Bath visits and shifts up to 24 hours for clients who require personal care assistance, including bathing, dressing, toileting and home exercise programs. Short or long term senior personal care services are available.

Senior Transportation. This service offers companions who can accompany people to appointments, adult day programs, community centers, church services, shopping, social events, and the like.

Reminder/Telephone Reassurance. Daily visits and telephone reminders may help people remember such items as appointments, dates, birthdays, anniversaries, medication and treatment times. A friendly phone companion can socialize, monitor safety, and make sure other needs are being met.

SWIFT® Wellness Program. This program is the “First of Its Kind” Non-Medical Elder Care Wellness Program. Wellness programs focus on the integration of mind, body, and spirit, allowing people to achieve their goals and find meaning and purpose in their lives.

Services: Alpha One manages three distinct home healthcare programs: the MaineCare Consumer Directed Assistance Services Program for consumers over 18 with physical disabilities who meet MaineCare’s financial and clinical eligibility, but are not nursing-home eligible; Home Based Care (HBC) Physically Disabled Program for individuals over 18 who are at risk of nursing home care and are over assets for MaineCare eligibility; and the Physically Disabled Waiver Program for those over 18 with significant physical disabilities who are financially and clinically MaineCare eligible.

Services: Living with a chronic disease can be challenging, but patients who take an active role in their own care can make more informed choices and are less likely to be re-hospitalized down the road. The Chronic Care Management programs focus on involving patients in the health care process so they can achieve more independence and a better quality of life.  They use current medical evidence to develop clinical and patient education and plans for patient care. Amedisys offers other services as well, including home health nurses, physical therapy, occupational therapy, speech-language pathology, medical social work, and hospice care.
